Listed below are the majority of applications required to submit for health department services. Please note that most applications will require a minimum of 30 days to process. Recommend you apply 45-60 days before you plan to apply for other permits (if applicable--like building permit).

• Camp Permit Application (New camps will require appointment with sanitarian)
• Environmental Health Survey (Obtained from licensing agency, like Child Care Adm. 410 819-5801)
• Food Permit (permanent facility) Application
• Food Permit (Special event-short term) Application
• Mobile Home Park Permit Application (New parks will require appointment with sanitarian)
• Perc Test Application *Deadline for wet season is December 1st--expect results end of wet season, usually June.
• Plat Evaluation Application (Obtained from Planning & Codes at time of plat submittal)
• Pool Permit Application (New pools will require appointment with sanitarian)
• Property Status Report Application (perc status)
• Rabies pre-registration form Not required to pre-register
• Records Search Request Application (use this for well/septic/perc inquiries)
• Septic Permit Application
• Sewage Contractor's License Application (Installer and/or Hauler)
• Water/Sewer Verification Application
• Well Application (Note: driller must contact the health department for the green form, but can print out the county form from this site)
• Workmen's Compensation Certificate of Compliance Application (Note: This application must be picked up from the health department)
