PLAT
CHECKLIST/PROTOCOL/BLOCK WORDINGS
It is
REQUIRED that before submitting plats for any project to check with the
Planning Office at 410 479 8100 to ensure compliance with zoning/codes
regulations. Also, if project involves property that has existing home
with private well and septic systems or farmland that has irrigation wells
then a Water and Sewer Verification
(AKA: WSV) must be processed before the plats can be submitted formally. WSV
is also required before perc applications can be processed in cases where
existing wells and septic systems are on the parcel of record.

PROTOCOL OF
FORMAL PLAT SUBMITTAL
-- Plats must be accompanied by fees and appropriate block wording must be on
plat at time of submittal.

HEALTH DEPARTMENT SUBMITTALS:
When submitting formal plats to health dept only; i.e. Planning Office
review not required, then those plats must have signatures of owner and
surveyor, and surveyors seal, as those plats went thru the
worksheet review process previously. The fee would have been addressed
on the worksheet--call 410 479 8045 if you need to verify fee due the health
department. A minimum of 3 plats must be submitted
to Health Department.
- PLANNING
OFFICE SUBMITTALS: Be prepared to pay two fees, one to Caroline
County Commissioners and one to Caroline County Health Department.
Planning Office will fill out the Plat Application form which you will take
to the Health Department--pay the fee noted on that form. Please note
that
plats being submitted to the Planning Office may need to go thru review
process a few times before final signatures required. Contact Planning
Office for their requirements as far as fees and number of copies due at
time of submittal.

Abandon an existing recorded SRA, submit to Health Department only

Change or add SRA, but not subdividing, submit to Health Department only

Combining parcels, submit to Planning Office*

Create new lot *(private septic and well), submit to Planning Office*

Create new lot *(public** septic and water), submit to Planning Office*

Establishing Sewage Reserve Area (SRA), but not subdividing, submit to
Health Department only

Line Revision (but not creating new lots), submit to Planning Office*

Site plan- for perc tests (also known as

Evaluation Test Location Plat)
*Check
with Planning at 410 479 8100 for formal plat submittal requirements and fees; and to
determine if Minor or Major subdivision plat.
**Public
systems will require Water and Sewerage Allocation Certificate and Capacity
Commitment for the Subdivision of Land from the town offering public system;
but the plats must be submitted to the Planning Office for final review.
