Text Box: This outreach program is for tracking the immunizations of all children living or born on Caroline County. All newborns receive a tracking booklet with tear off postcards that are returned to the health department for documentation of a child’s immunization status   Children should have completed their shot series by the age of 2.  Additional follow up may also be done by contacting the child’s health care provider or the parents.

Text Box: The TRAC (To Reach All Children) Project was created by the Caroline County Health Department, in cooperation with private health care providers, vendors, and Talbot, Queen Anne and Kent County Health Departments to ensure all children from birth to the age of two years receive complete and timely immunizations and are tested for lead at 12 & 24 months. The objectives include: 1) tracking the immunization status of all infants at their source of medical care. 2) educating parents about the importance of immunizations and lead testing through home visits and outreach activities, and 3) ensuring best practices regarding immunization delivery per schedule.
All children born or living in Caroline County receive the TRAC booklet with tear off cards that are returned to the health dept. for documentation of a child’s immunization status and lead test results.  Additional follow up may require that the health department contact the care provider or the parents.
