Please note: Birth and Death Certificates are only available from 8:30AM to 4:30PM.

Procedure for obtaining Copies
1) Appear in person at the Health Department.
2) Be the person referred to in the birth certificate, OR Be the parent of the person referred to in the birth certificate.
3) Have a birth date after 1939 ( Earlier birth records not yet available in database).
4) Have photo identification (Driver's License, or - call for details).
5) Complete form requesting a duplicate .
6) Pay $32.00 fee.

Death Certificates
Procedure for obtaining copies within 4 weeks of date of death:
1) Interested person appear "in person" at the Health Department.
2) Complete form requesting duplicate.
3) Pay $20.00 fee.
After 4 weeks of the Date of Death contact:
