The Caroline County Health Department sponsors the Caroline County Medical Adult Day Care Program. The purpose of this program is to provide comprehensive services to maintain and enhance ones quality of life in the least restrictive setting.
Criteria for Qualification
Age 16 or Older
Maryland Resident
Under the Care of a Physician
Have Health, Physical, or Mental Needs
At risk for Institutional Care
Transportation Available for County Residents.

eferrals can be made by friends, family, nurses, physicians, and potential patients.The Veterans Administration and Medicaid reimburse the Center; private pay fees are determined by a sliding scale according to ones income.
Attendance can be from two (2) to five (5) days a week

ADULT EVALUATION REVIEW SERVICES (formerly GES)

Adult Evaluation Review Services (AERS) is a program to assist adults that are at risk of losing their independence due to health, social or environmental issues. The AERS team (nurse and social worker) evaluates the client using a comprehensive assessment tool. The information gathered is used to develop an individual plan of care that links the client with appropriate support services. Referrals to various agencies and/or programs are made on the clients behalf.
When it is determined that a nursing home placement is necessary, AERS may complete the same assessment as a prerequisite for a Medicaid paid stay. AERS also assists in determining the least restrictive environment in which a client can effectively function.
An AERS comprehensive assessment is free of charge and is non-obligating. You must reside in Caroline County and need assistance with everyday tasks of living to be eligible for this service. We accept referrals from any source (e.g., hospitals, physicians, neighbors, churches, agencies, family members, friends and self).
MEDICAL ASSISTANCE PERSONAL CARE

The Medical Assistance Personal Care program helps qualified chronically ill and disabled persons to remain in their homes by providing assistance with some of the simple activities of daily living. These are activities such as bathing, dressing, grooming, feeding, moving about and toileting.
Medical Assistance pays a care provider to come to the home to assist with these activities. A registered nurse supervises the care provider. Home visits are made by the nurse periodically to see that the care is adequate.
To qualify for this program, the person must be chronically ill or disabled, be under a doctors care for chronic illness or disability, need assistance with personal care (bathing and hygiene) and be eligible for medical assistance.
