The Developmental Disabilities Administration – Resource Coordination/Targeted Case Management Program on Maryland's Eastern Shore is the single point of entry for services funded by DDA.  Services include residential, day, and support services for eligible individuals.  A Resource Coordinator coordinates services, monitors services rendered, and advocates on behalf of the individual with developmental disability. 

 

When you contact your county Resource Coordination, please be able to provide him or her with the following information about the person seeking services:

·             Name, address, and phone number

·             Date of birth

·             Social Security number

·             Insurance information (e.g. Medical assistance, Medicare, SSDI)

·             Type of service being sought (e.g. day, residential or support services)

·             Documentation of disability

·             Primary contact person and their phone number
