High blood pressure—also known as hypertension—is a common condition that significantly increases the risk of heart disease, heart attack, and stroke. To help adults take control of their heart health, the Caroline County Health Department is offering the Healthy Heart Ambassador Program, a structured, supportive program focused on blood pressure self-management and healthy lifestyle change.
The Healthy Heart Ambassador Program is designed for adults with hypertension who want practical tools, education, and guidance to help achieve and maintain healthy blood pressure levels. Through a combination of self-monitoring, lifestyle coaching, and nutrition education, participants will learn how small, sustainable changes can lead to meaningful improvements in overall health.
Program Benefits
Participants who enroll in the Healthy Heart Ambassador Program will receive:
- A 4-month program with personalized guidance and dedicated support
- Monthly 60-minute nutrition education seminars
- Tools to measure and track blood pressure at home
- Two individual consultations each month (10–15 minutes each)
- Ongoing education on managing blood pressure, cholesterol, and blood sugar
All meetings include blood pressure self-management and nutrition education to help participants build confidence and long-term healthy habits.
Eligibility Requirements
To participate, individuals must:
- Be 18 years of age or older
- Live or work in Caroline County
- Have been diagnosed with high blood pressure or currently take antihypertensive medication
- Not have experienced a cardiac event within the past 12 months
- Not have atrial fibrillation or other arrhythmias
- Not have or be at risk for lymphedema
