COORDINATOR SPECIAL PROGRAMS HEALTH SERVICES II, DD
Filing Deadline: October 7, 2026
Wage Range: $59,305.00 – $76,725.00/year, with potential growth to $93,533.00/year
Employment Type: Full-Time
The main purpose of this position is to coordinate services provided to individuals served by the Developmental Disabilities Administration (DDA) through the Developmental Disabilities Program (DDP) Targeted Case Management (TCM) and Coordinator of Community Services (CCS) programs. This position will coordinate available community resources for eligible individuals with developmental disabilities who are residents of Maryland’s Eastern Shore. This position will assess and determine eligibility for DDA services and provide referrals for those services. This position will arrange access to day, residential and other support services. This position will facilitate annual planning meetings and assist individuals and their families with developing a plan of service. This position will develop the Person-Centered Plan (PCP) and conduct periodic monitoring of the PCP and satisfaction with delivered services. This position will follow individuals on the waiting list registry and assist with transition planning. This position will maintain accurate documentation of all services provided. These efforts will support individuals in maintaining community living, promoting individual choice and improving overall quality of life.
Per COMAR Regulation 10.09.48.05E a Coordinator of Community Services shall have:
(a) A Bachelor’s degree from an accredited college or university in a Human Services field; OR
(b) An Associate’s degree with two (2) years experience in a Human Services field; OR
(c) Seven (7) years experience in a Human Services field.
COMMUNITY HEALTH NURSE II: MATERNAL CHILD HEALTH COMMUNITY NURSE

Wage Range: $39.20 – $50.71/hour, with potential growth to $61.81/hour
Employment Type: Full-Time Contractual
The main purpose of this full-time contractual position is to implement maternal and child health grants at the local health department (LHD) level. This position will provide case management services to pregnant and postpartum community residents. This position will assist with tracking grant-associated performance measures and identify realistic opportunities for program growth within the LHD’s capabilities. This position will also assist with other nursing duties, including emergency preparedness response and vaccination clinics, as needed.
MEDICAL CARE PROGRAM ASSOCIATE II

Wage Range: $49,214.00 – $63,669.00/year, with potential growth to $77,620.00/year

The main purpose of this position is to determine eligibility for and assist individuals and families with the eligibility process for the MCHP, FAC and MA Programs.
